The Top Surgery Process

Whether MTF or FTM, top surgery creates results that are permanent. Here’s what you can anticipate:

MTF

Although MTF top surgery usually uses breast implants, it’s a bit different than typical breast augmentation for cisgender women. This is because transgender women may not always have enough natural tissue to cover their desired implants. If you’d like implants that are on the large side, your surgeon may recommend a temporary tissue expander to help your body produce enough skin to accommodate the implant fully. If you desire a subtler result, however, a tissue expander may not be necessary, especially if you’ve been on estrogen for some time and you’ve developed your own natural breasts.

MTF top surgery is performed while you’re under general anesthesia. Using an incision placed around your areolae or within the breast crease, the surgeon makes a pocket within the pectoral muscle and places the implant within this pocket. He then closes the incisions with sutures.

FTM

FTM top surgery is similar to a traditional breast reduction, but it is more technically complex. While you are under general anesthesia, Dr. Calobrace or Dr. Quintero performs a mastectomy to remove existing breast tissue completely. He then sculpts the remaining tissue into a masculine-looking chest. Finally, he positions the nipples and areolae, and, if necessary, raises them higher on the chest or reduces their diameter. The final step is to close your incisions with sutures.

What to Expect During Recovery

Both MTF and FTM top surgeries are outpatient procedures. Your surgeon provides you with recuperation guidelines and instructions, depending on the specifics of your own surgery. However, you can expect to be away from work and other daily activities for about 1 week, and potentially longer if you work in a physically demanding job. You should avoid strenuous activity, especially that which requires upper body exercise, for at least 6 weeks after your top surgery. MTF patients should be aware that their implants will go through a period of acclimation, during which the implants will soften and drop to a natural-looking position.

Breast implants do not last forever, but they have long “lifespans”–typically exceeding a decade. The results of FTM top surgery are permanent. In either case, you’ll be able to enjoy the body you’ve envisioned for many, many years.
